SELF-IDENTIFICATION INVITATION TO INDIVIDUALS WITH DISABILITIES AND PROTECTED VETERANS

In accordance with the Americans With Disabilities Act, Sections 503 of the Rehabilitation Act of 1973 (as amended), Section 4212 of the Vietnam Era Veterans Readjustment Assistance Act of 1974 (as amended in 2002) and all subsequent amendments and regulations issued to protect the rights of Individuals with Disabilities and Protected Veterans and spouses of veterans and the commitment of Terumo Cardiovascular to provide reasonable accommodations for and assurance of non-discrimination toward qualified individuals with disabilities and Protected Veterans, it is policy that:
